You are required to contact Care One when you must call off of work. You can call or email your Care One recruiter. Care One monitors an afterhours phone 24/7/365. The number is 734-794-3792. If you need to leave a message follow the prompts.
You are also required to contact your MDOC supervisor when you must call off of work. You are responsible to obtain the necessary contact information of who to contact at the facility, in the event of an unscheduled absence.
You must call in every day that you are unable to work. A doctor’s note for extended time off needs may be requested.
All employees are expected to maintain what in Care One’s judgment, is a professional
manner and appearance at all times. Toward this end, among the standards that ALL
Care One employees MUST FOLLOW are:
1. Clinical Staff are to wear scrubs in any color EXCEPT BLUE.
Mental Health staff are permitted to wear business casual attire.
2. Employees must always wear a well-fitting, clean, wrinkle-free uniform. There are to be no
missing buttons, sagging hems or tears in any part of your uniform;
3. Employees are expected to be properly attired for the site to which they are
assigned. Other than rings and non-hoop or non-dangling earrings, jewelry
should not be worn while on duty. If management determines that your attire is
not appropriate, then you may be relieved of duty without pay;
4. Your hair should be neat and clean. Shoulder length hair or longer should be
pulled up and away from your face.
5. Your nails must be clean and free of chipped nail polish; nails should be no longer than ¼ inch
5. You must ALWAYS wear your nametag on assignments;
6. You should arrive at your assignments sufficiently before your shift begins so that
you are fully ready to start work, in uniform, at the schedule start of your shift.
Your badge, parking pass and all other department issued materials or equipment must be returned at the end of employment, prior to your last direct deposit.
If your badge is not received before direct deposits have been processed, you will be issued a live check which can be picked up in our Ypsilanti office, when your badge has been returned.
We are required to confirm that your badge has been returned due to safety and security concerns at the facilities.
You may leave it at the facility on your last day. If you chose to do this, it is strongly encouraged that you remember the name of the person you are returning it to. We must confirm that the badge can be accounted for and will need to follow up with the facility.
Alternately, you may return it to our Ypsilanti office before your final direct deposit.

Refer to your Formal Offer Letter to find the address for the training location. This may be different than the facility where you will be working.
Review the Allowable Items list to be sure you are not taking anything inside that could be considered contraband. If you are unsure, leave the item in your car until you can confirm if it is allowed inside the facility.
Allow yourself plenty of time to get to the training center and through security.
The first week of training is scheduled for Monday- Friday 8:00 am-4:30 pm.
You will use your driver’s license as your identification until a work badge has been provided.
When you arrive at the facility, inform security that you are a new hire and will be attending training. They will advise you where you need to go.
During training, you will be assigned an email account and will enroll in MI TRAIN where you will complete all computer based training modules.
When you have completed all of the modules, you are required to provide Care One with a transcript of completion.
You will meet with the scheduler or other supervisor to receive your regular schedule during this week.
Be sure to inform the scheduler/supervisor of any pre-planned time off needs that you may have (appointments, vacations etc.)
It is suggested that you obtain the email address and phone numbers for your Health Unit Manager, direct supervisor, scheduler, and other important facility personnel in the event that you need to reach them outside of work hours.

An employee must work ON the holiday to be eligible for holiday pay (not the “observed” day).
Holiday pay will be paid up to 8 hours only. Any hours worked over 8 hours may be paid at straight time, unless it is overtime.
In general, if the office where you are assigned is CLOSED on the holiday, you may not be scheduled to work.

In general, if a holiday falls on your regularly scheduled work day, you may be scheduled to work.
You will have an opportunity to make requests for time off when the vacation/holiday book is made available at each facility.
Care One does not have the ability to make or change the holiday schedule.
If you have specific scheduling requests, you must coordinate with your Health Unit Manager (HUM)
Care One offers health insurance, dental insurance and supplemental insurance AFLAC policies
You will receive information regarding all insurance policies after about 6 weeks of employment.
You will be eligible for coverage to begin at 90 days.
Rates are determined according the ACA guidelines, which is income based.
Deductions for health and dental policies are prepaid a month ahead of the coverage.
If you decline health coverage at 90 days, you may not be eligible to enroll until the next open enrollment period, which occurs every year in November.
Pay periods are every other Monday. We offer both weekly and biweekly pay options.
Your first pay check will be received three weeks after your start date. (see your formal job offer letter for your first pay date). This is due to the way the pay periods fall and when authorized time sheets/punch sheets are available.
If you have chosen the weekly pay option, you will be paid for the first week of work only, even if you have worked both weeks in the pay period. Thereafter, you will receive a weekly check.

A BI WEEKLY pay period is 1/1/23-1/14/23 and will be paid on 1/23/23.

The first week of this pay period is 1/1-1/7/23 and will be paid on 1/23/23
The second week of this pay period is 1/8-1/14/23 and will be paid on 1/30/23.
Please fill out a Care One time sheet and submit them every two weeks, following the MDOC pay schedule. It is imperative that you submit a Care One time sheet in the event that there are discrepancies with the punch sheets generated at the facility such as the time clock is not working, you forget to punch in/out, work through a lunch period, etc. (it happens all the time!)
We will do our best to confirm any discrepancies, however if we don’t have a time sheet from you, we have nothing to compare and will have to defer to the facilities documentation. This will also cover you in the event that a punch sheet is not submitted from the facility.
In 1-2 weeks after you have started work, you will receive an electronic invitation from Quickbooks/Intuit to set up an online portal. Here you will be able to view all of your pay information, including pay stubs and W2’s at the end of the year. The invitation expires after 48 hours. If you are unable to setup your online portal within that time period, please contact us to have another link sent to you.
***If you have ever used an Intuit/Quickbooks/Turbo Tax account for any other purpose, do not create a second account. You can access your Care One information using the account you have already established.

Accrual of 1 hour earned for every 35 hours worked begins on the first day of employment.
PTO cannot be used until an employee has completed 26 weeks of employment.
PTO may not exceed 40 hours per year.
PTO cannot use paid time off in the last two weeks of employment.
You may use PTO for any time off needs, including vacation and sick time.
You should write your request to use PTO time on your Care One time sheet. You should write how many hours wish to have applied.
PTO cannot be used if it will cause overtime.
PTO balances can be checked by contacting the office at 734-794-3792.
Care One is offering a $500.00 referral bonus to all of our current full-time employees, who refer a qualified candidate for full time work, to Care One.
The referred candidate must be hired and work a full-time schedule for 12 weeks with Care One.
The referring employee must remain employed with Care One and be in good standing to qualify for this bonus.

SCHEDULES:
Schedules are made based upon the needs of each individual facility/unit. All scheduling issues must be discussed with the facility scheduler/supervisor including time off, vacation requests and any other specific scheduling needs.
Care One does not create, monitor or otherwise manage employee work schedules. We cannot make, change, approve or deny any scheduling needs.
Some positions may include weekend and holiday rotations.
There may be occasions when you may be asked to work an alternate schedule. This could include a change in the needs of the facility, emergency situations, staffing shortages or similar.
If this is outside of your contracted hours, your supervisor will likely discuss this with you as far in advance as practical. It may only be a temporary change. They will try to work with each employee to accommodate their needs whenever possible.

A punch sheet will also be generated at the facility. Your supervisor will approve, sign and send these sheets to Care One. This punch sheet will be compared to your Care One time sheet for accuracy. (You are not required to do anything about this, this is just informational so you are aware of the process.)
NEW HIRE TRAINING
The first week of work is for new hire training. It is scheduled Monday- Friday 8:00am -4:30 pm. You must attend all training sessions and complete all training modules.
During training, you will be provided with a work badge. This badge will also be used for security purposes and to clock in/out. It may take several days to receive the badge. Until then you will use your driver’s license.
Training transcripts are required to be sent to Care One upon completion. Your trainer should be able to assist if you have any difficulty retrieving the completed transcripts. The transcript reports should look like one of the samples below:
ANNUAL TRAINING
Employees are required to complete annual training every year. The transcripts for annual training are due to Care One by September 1st of each year.
Care One will inform you when these modules have been released to MI TRAIN via your personal email address and your work email address.
It is imperative that you complete these trainings as quickly as possible and submit an updated transcript. (Certificates are NOT necessary, the transcript contains all of the information needed.)
Because there are many modules, it is suggested that you do not wait until the last minute to work on these trainings.
You are permitted adeuqate work time to complete these modules. If you are unable to get adequate time to work on these during your work hours, please inform Care One as soon as possible.
SPECIAL TRAINING
From time to time, there may be new training modules, in person trainings, or virtual training added to the requirements.
Any new trainings that are added, will be communicated to you via your personal email address and your work email address.
It is imperative that you complete these trainings as quickly as possible and submit an updated transcript. (Certificates are NOT necessary, the transcript contains all of the information needed.)
